In-home care at no extra cost

Blue Shield has teamed up with ConcertoCare to provide medical care and support services in members’ homes. This is an optional program designed to work hand in hand with a member's existing primary care doctors and specialists. Eligible members may be contacted by ConcertoCare by letter, phone, email, or text.

What is ConcertoCare?

ConcertoCare is an in-home provider that delivers medical care and support services in addition to your plan’s benefits and services. With ConcertoCare, members keep their current primary care doctor, specialists, and all the benefits available from Blue Shield.  

 

Who qualifies for in-home care?

Blue Shield members aged 18 or older in certain locations who are managing multiple chronic illnesses or need extra support to manage their health.

Members that qualify for this program may hear from a ConcertoCare representative by mail, phone, email or text.

What kind of help can I receive? 

ConcertoCare works with members’ primary doctors and assembles a team to help coordinate care. ConcertoCare will be there for you 24/7, virtually or in person. Services provided by ConcertoCare include:    

  • Urgent and acute care services  
  • Mental health services  
  • In-home and virtual care appointments  
  • In-person lab services  
  • Medication management  
  • Coordination of transportation services  
  • Nutrition guidance  
  • Chronic illness management  
  • Home assistance and other services  

What kind of providers are available?

The ConcertoCare team includes the following professionals, who are available for care and services::

  • Doctors
  • Advanced practitioners
  • Registered nurses 
  • Social workers 
  • Health coaches 
  • Medical assistants 
  • Pharmacists

Are there really no additional costs?

As long as you qualify for the program, there are no additional costs for services provided by ConcertoCare.*

ConcertoCare is independent of Blue Shield of California and is contracted by Blue Shield to provide in-home and virtual care services.   
These services are not covered benefits of Blue Shield health plans and none of the terms or conditions of Blue Shield health plans apply. Members who are not satisfied with products or services received from ConcertoCare may use Blue Shield's grievance process described in the Grievance Process section of the Evidence of Coverage or Certificate of Insurance/Policy. Blue Shield reserves the right to terminate this program at any time without notice.
